Day shifts, 5 days a week, with potential variations in start times (07:00 - 17:00). Weekend rotation required every other weekend.

Job Description

The Case Manager RN will be responsible for coordinating patient care and discharge planning in an acute hospital setting, ensuring high-quality outcomes for patients. This role requires effective communication with interdisciplinary teams to manage patient needs effectively.

Responsibilities

• Manage patient care coordination and discharge planning.
• Conduct admission criteria assessments and determine medical necessity per evidence-based guidelines.
• Review continued stay and pre-certification for medical necessity.
• Ensure compliance with HIPAA and CMS regulations.
• Collaborate with interdisciplinary teams including social services, pharmacy, and case managers to ensure comprehensive care.

Required Experience / Certifications / Licensure

• Valid Alaska RN license in hand required.
• Minimum of 1 year of acute care experience required.
• Epic experience is required.
• BLS certification preferred.
• ACM Certification is preferred.
