Acute Care Case Manager

Schedule

Day shift, 5x8-hour schedule from 08:00 AM to 04:30 PM, with weekend rotation required.

Job Description

We are seeking a dedicated Acute Care Case Manager with a minimum of 2 years of experience in acute care case management and discharge planning. The ideal candidate will have robust clinical skills and the ability to manage a patient load of up to 25 patients per day. This position requires strong coordination and discharge planning abilities, as well as adherence to patient care standards.

Responsibilities

• Conduct assessments and develop treatment plans for patients.
• Coordinate care and facilitate smooth discharge planning.
• Participate in interdisciplinary team meetings to discuss patient care.
• Work with various healthcare professionals to ensure quality care delivery.
• Maintain accurate and timely documentation using computerized systems.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in acute care case management or discharge planning.
• Valid California state license required; pending licenses accepted if active by start date.
• Experience with care coordination, utilization review, and discharge planning.
• Knowledge of Medicare and Medicaid regulations is essential.
