Job Description

We are seeking a dedicated Home Health Registered Nurse with recent experience in home health care. The role involves providing high-quality nursing care to patients and ensuring their comfort and safety during home visits. The ideal candidate will have experience with HomeCare HomeBase (HCHB) and be familiar with OASIS assessments.

Responsibilities

• Conduct OASIS assessments and complete necessary documentation.
• Manage patient care plans and ensure compliance with treatment protocols.
• Conduct routine visits and provide education to patients and families.
• Complete discharge planning and recertification visits as required.
• Meet productivity expectations, maintaining an average of 28-32 points per week.

Required Experience / Certifications / Licensure

• Recent experience in home health care is required.
• HomeCare HomeBase (HCHB) experience is preferred; online training is available if not currently experienced.
• Active RN license is required.

Location

Candidates must be able to cover the Forsyth & N. Hall, Dawson, Lumpkin, and Whitern areas.
