Part-Time Licensed Practical Nurse (LPN) - Nights

Schedule

This is a part-time position requiring 24 hours of work per week with 2 eight-hour shifts one week and 3 eight-hour shifts the following week. Every other weekend on Friday and Saturday nights is required.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents. The individual will demonstrate the knowledge and skills necessary to provide care appropriate to adult and geriatric patients, ensuring compliance with established policies and procedures under the supervision of a Registered Nurse.

Responsibilities

• Collect and communicate ongoing physical and psychosocial condition data of residents to the RN and interdisciplinary team.
• Contribute to creating developmentally age-appropriate and culturally relevant care plans.
• Recommend revisions to care plans based on resident outcomes and collaborate with the healthcare team to ensure comprehensive care.
• Maintain current knowledge and competency in nursing practices and participate in staff mentoring and orientation.
• Ensure safety, cost-effectiveness, and positive impact in care delivery.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• Basic Life Support (BLS) certification (AHA or RQI only accepted)
• A minimum of 1 year of experience as an LPN, preferably in a Long-Term Care (LTC) facility
