M: 6:30 AM - 12:30 PM
T/TH: 9:00 AM - 7:00 PM
W: 6:30 AM - 3:00 PM
F: 7:00 AM - 3:30 PM

Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician direction. The therapist will ensure the design of functional treatment programs allowing for effective community reintegration. This role includes educational responsibilities to patients, families, students, coworkers, and self, along with adherence to organizational compliance and professional standards.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals focused on patient progress.
• Document medical records accurately and timely, complying with state, federal, and clinic guidelines.
• Participate in the student program, providing learning opportunities through supervision and evaluations.
• Create an individualized professional development plan and pursue ongoing education.
• Adhere to state practice acts and maintain professional licensure in good standing.
• Engage in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an Accredited School
• 6 months of outpatient clinical experience
• Valid CPR Certification
• Active licensure applicable to the state of practice
• Previous assignments with the organization are highly desired
