Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans tailored to the patients' needs, goals, and physician directions. Responsible for the delegation and direction of discipline-specific therapy services, the therapist ensures the design of functional treatment programs for an effective return to community activities. The role entails serving as an educator to patients, families, students, and coworkers while upholding the organization's mission and compliance standards.

Responsibilities

• Completes comprehensive evaluations of assigned patients.
• Utilizes clinical reasoning to develop treatment plans and goals focused on patient discharge.
• Documents medical records accurately and in a timely manner in alignment with standards.
• Ensures care is provided under valid physician orders and within insurance approvals.
• Participates in student programs, providing supervision and learning objectives.
• Actively pursues professional development through continuing education and self-study.
• Adheres to the discipline-specific State Practice Act and maintains professional licensure.
• Contributes to performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license for physical therapy.

Schedule

This position operates on a 5x8.0 schedule.
