Outpatient Physical Therapist

Job Description

We are seeking a qualified Outpatient Physical Therapist to join our team. The therapist will be responsible for evaluating patients and creating a treatment plan tailored to their needs, goals, and physician direction. The role will involve guiding and directing therapy services to ensure optimal recovery and functional return to community activities.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop, implement, and modify treatment plans based on patient assessments.
• Ensure that care aligns with physician orders and complies with regulatory guidelines.
• Document patient care in a clear, concise, and timely manner.
• Educate patients, families, and staff about treatment plans and goals.
• Participate in the supervision and development of students when applicable.
• Stay current with continuing education and maintain professional licensure.
• Adhere to safety protocols to ensure a safe environment for patients and staff.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited program.
• Minimum of 1 year of outpatient clinical experience.
• CPR Certification.
• Active Physical Therapist license for the state where services will be performed.

Schedule

This position operates on a 5x8.0 schedule.

Assignment Duration

This is a 13-week contract position.
