Physical Therapist - Rehabilitation

This position offers flexible 8-hour day shifts, Monday to Friday, with consistent communication regarding times with the Director of Rehabilitation.

Job Description

The Physical Therapist will effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care, while developing appropriate plans of care that adhere to regulatory and clinical practice standards.

Responsibilities

• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Assess patient progress and adjust treatments accordingly in compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluations.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Maintain effective communication with supervisors and healthcare team members regarding patient progress and treatment plans.
• Document all necessary information in the electronic documentation system while ensuring compliance with regulations.
• Participate in required meetings, trainings, and conferences related to patient care and rehabilitation.
• Supervise and support physical therapist assistants and rehabilitation aides as needed.
• Adhere to APTA and state practice acts, as well as codes of conduct and ethics in professional practice.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience as a Physical Therapist in Long-Term Care (LTC) or Rehabilitation settings.
• Current Missouri State License as a Physical Therapist.
• New graduates are welcome if they possess Level II experience.
