Multi-Modality Physical Therapist

This is a Monday to Friday day shift position, typically 8 hours, but 4x10 shifts may be considered.

Job Description

We are looking for a dedicated and experienced Multi-Modality Physical Therapist to provide care in various settings, including home health, outpatient, and acute facilities. The therapist will be responsible for delivering high-quality rehabilitation services while working at facilities within a 60-mile radius. Responsibilities include evaluating and treating patients with diverse conditions, ensuring effective communication with healthcare teams, and maintaining accurate patient records.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Provide therapy interventions for post-operative, sports injury, and geriatric patients.
• Perform home health visits and outpatient therapy sessions.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Document patient progress and treatment outcomes accurately.

Required Experience / Certifications / Licensure

• Valid Physical Therapist license.
• Strong general experience in outpatient and inpatient settings.
• Experience treating post-operative, sports injury, and geriatric populations.
