Job Description

We are seeking a dedicated Physical Therapist to join our Home Health team. In this role, you will provide services to patients in their personal homes or assisted living facilities. You will be responsible for conducting comprehensive assessments, creating treatment plans, and collaborating with a multidisciplinary team to deliver high-quality patient care.

Responsibilities

• Conduct comprehensive assessments/evaluations and develop treatment and care plans.
• Provide oversight and guidance to Physical Therapy Assistants.
• Manage a caseload to ensure effective patient care, with an expected workload of approximately 6 visit equivalents per day.
• Work with a multidisciplinary team including other therapists, nurses, and social workers.

Required Experience / Certifications / Licensure

• Current North Carolina license to practice Physical Therapy.
• BLS/CPR certification.
• Experience in completing OASIS assessments and home health care.
• Preferably possess knowledge of the new payment model (PDGM) effective January 1, 2020.
