Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients, developing, implementing, and modifying treatment plans in accordance with the patient's needs, goals, and physician direction. This role is responsible for delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective community reintegration. The therapist serves as an educator for patients, families, students, coworkers, and themselves while upholding professional standards and the organization’s mission statement.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals aimed at efficient progression towards discharge.
• Document medical records clearly, concisely, and accurately, adhering to state, federal, and clinic guidelines.
• Participate in student programs, providing a positive learning environment and appropriate supervision.
• Formulate and pursue an individualized professional development plan with clinical management.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• CPR certification.

Schedule

This position follows a 5x8.0 schedule.
