Job Description

We are seeking a Registered Nurse with experience in skilled nursing facilities (SNF) and proficiency in the Rehab Optima (NetHealth) computer system. The ideal candidate will be responsible for providing high-quality patient care and ensuring effective management of nursing practices within the facility.

Responsibilities

• Administer nursing care to patients in a skilled nursing facility setting.
• Manage patient assessments, care plans, and medication administration.
• Utilize the Rehab Optima (NetHealth) system for documentation and management of patient data.
• Collaborate with multidisciplinary teams to optimize patient outcomes.

Required Experience / Certifications / Licensure

• Active California RN license.
• Experience in a skilled nursing facility.
• Knowledge of Rehab Optima (NetHealth) computer system preferred.
