Schedule

Day shift, 5x8 hours (08:00 - 17:00). Every 3rd weekend required.

Job Description

We are seeking a dedicated Home Health Registered Nurse (RN) with experience in managing patients with various conditions. The ideal candidate will perform skilled home health visits, provide assessments, and collaborate with patients and their families to educate them on care plans and medications.

Responsibilities

• Conduct assessments and document patient health status.
• Administer medications and manage patient care plans in home settings.
• Provide patient and family education on health management and safety.
• Utilize home health equipment and devices effectively.
• Report and document any signs and symptoms of abuse or neglect.
• Participate in the OASIS documentation process.
• Adhere to safety protocols and ensure the physical well-being of patients.

Required Experience / Certifications / Licensure

• Must have a valid RN license for Florida or a compact state license.
• BLS certification is required.
• 2 years of experience in home health preferred.
• OASIS training is preferred.
• Experience with cardiopulmonary, geriatrics, neuro, and ortho patients is necessary.
• Must possess a valid driver's license and reliable transportation for home visits.
• COVID-19 vaccination or an approved exemption is required.
• Flu vaccination or declination required, with mandatory mask-wearing protocols in place.
