Job Description

We are seeking a skilled Occupational Therapist to join our home health care team. This position involves providing care to patients with a variety of diagnoses, focusing on rehabilitation and daily living activities. You will be responsible for evaluating patients, developing and implementing treatment plans, and educating patients and families to enhance recovery and quality of life.

Responsibilities

• Conduct evaluations and assessments of patients' needs and capabilities.
• Develop individualized treatment plans and goals in collaboration with patients and families.
• Perform interventions focused on improving patients’ abilities to perform daily living activities.
• Utilize therapeutic modalities such as TENS and ultrasound in treatment.
• Educate patients and their families on rehabilitation techniques and home exercises.
• Document patient progress and adhere to billing and OASIS requirements.
• Coordinate with interdisciplinary team members to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum 1 year of home health experience required; 2 years preferred.
• Active state occupational therapy license required.
• Basic Life Support (BLS) certification required.
• OASIS training preferred.
• Must have a valid driver’s license and a reliable vehicle for home visits.
• COVID-19 vaccination required or exemption form on file.
• Flu vaccination required or declination with mandatory mask-wearing at work.
