Job Description

We are seeking a dedicated Home Health Registered Nurse to provide skilled care and support to patients in their homes. The ideal candidate should have experience in home health care and is familiar with OASIS documentation.

Responsibilities

• Conduct home skilled visits for patients with various medical conditions, ensuring quality patient care.
• Document patient visits using BayCare Forms and documentation systems.
• Manage a caseload of 6-8 patients per day, ensuring a minimum of 6 points per day at the manager's discretion.
• Coordinate care with interdisciplinary teams, including physical therapy and social services.
• Educate patients and their families on health management and ADLs.

Required Experience / Certifications / Licensure

• Must have a valid RN license in Florida or a compact state license.
• 1 year of home health experience is required.
• BLS certification is required.
• OASIS training is preferred.
• Must have a valid driver's license and a reliable vehicle for home visits.
• COVID-19 vaccination required, or exemption form on file; flu shot is mandatory.
