Job Description

We are seeking a skilled and compassionate Home Health Registered Nurse to provide high-quality care to patients in their home environment. As a Home Health RN, you will play a crucial role in assessing patients' needs, documenting care, and implementing a home exercise program tailored to the patient's condition.

Responsibilities

• Conduct patient assessments, focusing on cardiopulmonary, neurological, and orthopedic needs.
• Document patient care accurately and timely.
• Utilize appropriate equipment and devices, including gait belts, walkers, and supplemental oxygen.
• Provide patient and family education regarding medications, safety, and signs of abuse.
• Develop individualized care plans and home exercise programs.
• Assist patients with activities of daily living (ADLs) and monitor vital signs.

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health experience required.
• OASIS training preferred.
• Current state nursing license required.
• BLS certification required.
• Valid driver's license and reliable transportation for home visits.
• Must be COVID-19 vaccinated or have a valid exemption.
• Must have a flu shot or valid declination, with mandatory mask-wearing while at work.
