MDS Coordinator - Skilled Nursing Facility

This is a 13-week contract position.

Schedule

Day Shift: 5x8-Hour from 08:30 AM to 05:00 PM, Monday to Friday. No call or weekend requirements.

Job Description

We are seeking an experienced MDS Coordinator to join our interdisciplinary team within a Skilled Nursing Facility. The ideal candidate will be integral in ensuring compliance with federal and state regulations, overseeing MDS completion, and facilitating care plan meetings.

Responsibilities

• Complete MDS assessments and baseline care plans.
• Conduct care plan meetings and ensure proper development and completion.
• Oversee certification and recertification processes.
• Ensure clinical documentation is accurate and up to date.
• Manage admission referrals and preauthorization of insurance for new admissions.
• Maintain compliance with CMS, CPT coding, and ICD-10 coding regulations.
• Conduct utilization reviews and audits for clinical documentation improvement.

Required Experience / Certifications / Licensure

• Bachelor of Science in Nursing (BSN) with a minimum of 3 years of experience in MDS.
• Current North Carolina state nursing license required.
• Certification in Basic Life Support (BLS).
• Experience with Long Term Acute Care/Skilled Nursing is essential.
• Knowledge of regulatory requirements and MDS processes is mandatory.

Our unit serves patients with a variety of diagnoses including generalized weakness and surgical aftercare, providing short-term rehabilitation services.
