Physical Therapist - Home Health

This is a 13-week contract position.

Schedule

Day Shift: Monday to Friday, 8 AM to 5 PM. Flexibility may allow for a Tuesday to Friday schedule if needed.

Job Description

We are seeking a dedicated Physical Therapist for a home health position. The role involves managing a caseload of patients in their homes, providing evaluations, developing treatment plans, and implementing therapy interventions. You will be responsible for achieving productivity expectations and ensuring quality patient care.

Responsibilities

• Conduct evaluations and develop comprehensive treatment plans for patients.
• Administer therapy interventions according to established plans.
• Manage a caseload of 8-11 patients per day, considering travel time.
• Meet productivity expectations of 4-5 patients per day.
• Document patient progress using the Meditech electronic medical records system.
• Collaborate with a small team of therapists for interdisciplinary support.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience as a Physical Therapist.
• Current Missouri PT state license (PT compact privileges possible).
• Basic Life Support (BLS) certification required.
