Licensed Practical Nurse (LPN) - Long-Term Acute Care (LTAC)

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) for a Long-Term Acute Care (LTAC) facility. The ideal candidate will provide essential nursing care and cover Certified Nursing Assistant (CNA) duties as needed while ensuring high standards of patient care and safety.

Responsibilities

• Administer medications and perform nursing procedures as per established protocols.
• Assist with patient activities of daily living and monitor vital signs.
• Collaborate with healthcare team members to create and implement care plans.
• Ensure compliance with healthcare standards and regulations.
• Participate in patient education and support initiatives.

Schedule

This is an 8-hour Evening shift position with a rotating schedule that includes every other weekend. There are no on-call requirements or self-scheduling; however, opportunities to pick up extra shifts may be available.

Required Experience / Certifications / Licensure

• A minimum of 1 year of experience as an LPN is required.
• Current AHA BLS certification is mandatory.
• Experience in long-term care settings is preferred.
• Must have a New York state nursing license on file at the time of submission.
