Job Description

The Physical Therapist plans and conducts segments of treatment programs aimed at restoring and improving physical, social, and mental functions while adhering to departmental objectives. This role involves participating as a co-leader in an interdisciplinary approach to patient groups, conducting thorough evaluations to ensure quality standards, and addressing areas needing improvement.

Responsibilities

• Utilize appropriate clinical techniques and tools to establish personalized treatment plans for patients.
• Provide therapy interventions following established treatment protocols and techniques.
• Develop individualized care and discharge plans, ensuring effective communication with relevant parties.
• Complete comprehensive and accurate clinical documentation within designated deadlines.
• Supervise assigned therapy assistants as needed.
• Perform additional job-related duties as assigned.

Required Experience / Certifications / Licensure

• 2 years of Physical Therapist experience - Required
• Hawaii state Physical Therapist license - Required
• Bachelor's degree in a relevant field - Required; Master's preferred
• 1 year of home health experience - Required
• Epic experience - Required
• Travel experience - Preferred
• Current CPR/BLS certification - Required
