Licensed Practical Nurse (LPN) for LTAC

Job Description

We are looking for a dedicated Licensed Practical Nurse (LPN) for a Long-Term Acute Care (LTAC) position. The ideal candidate will be responsible for providing high-quality nursing care and covering Certified Nursing Assistant (CNA) duties as needed. This position requires a commitment to patient care and teamwork in a dynamic environment.

Responsibilities

• Provide nursing care to patients in a Long-Term Acute Care setting.
• Assist with daily living activities and monitor vital signs.
• Support CNA duties as required to ensure optimal patient care.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Participate in patient documentation and maintain accurate health records.

Schedule

This is an 8-hour evening shift position. The work schedule includes a minimum of every other weekend, with adjustments made as needed to meet the needs of the unit and patient care. No call or self-scheduling is required, but there may be opportunities to pick up additional shifts.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience as a Licensed Practical Nurse (LPN) is required.
• Current AHA BLS certification is required.
• Licensure as an LPN in the state of New York is required.
• All work history information, including bed count, EMR, and facility type, must be complete for submission.
• Must pass a 7-year background check, FACIS 3 check, and New York State check.
