Physical Therapist - Outpatient Rehabilitation

Schedule

Day shift, Monday through Friday, from 8:00 AM to 4:30 PM.

Job Description

The Registered Nurse will be responsible for providing quality nursing care, collaborating with interdisciplinary teams, and floating as needed between outpatient, medical-surgical, emergency department, and geriatric behavioral health settings. The role primarily focuses on those within the Yingling Outpatient Rehabilitation Center.

Responsibilities

• Provide acute care and rehabilitation services to adults and geriatric patients.
• Perform assessments and implement individualized care plans.
• Collaborate with physical therapy, social services, and rapid response teams.
• Monitor and manage patient care activities and vital signs.
• Educate patients and families on care plans and discharge instructions.
• Utilize adaptive devices and equipment to assist in patient rehabilitation.

Required Experience / Certifications / Licensure

• BLS certification required.
• Previous charge nurse experience preferred.
• Experience with the Cerner charting system is preferred.
• Must complete Pennsylvania fingerprinting as part of the credentialing process.
