Job Description

We are seeking a dedicated Home Health Registered Nurse (RN) to provide exceptional nursing care in our coverage territory of Northwest Philadelphia and Montgomery County. In this role, you will be responsible for executing comprehensive care plans, including start of care visits and revisits, ensuring all OASIS documentation complies with regulatory standards.

Responsibilities

• Conduct start of care assessments and regular revisits for home health patients.
• Utilize EPIC system for documentation and patient management.
• Complete OASIS assessments accurately and timely.
• Develop, implement, and evaluate individualized patient care plans.
• Collaborate with interdisciplinary teams to ensure holistic patient care.
• Educate patients and their families about care plans and health management strategies.

Required Experience / Certifications / Licensure

• Recent home health experience is required.
• EPIC experience preferred.
• OASIS certification is required.
• Current and valid RN license.
