LPN Full-Time Position

Schedule

This is a full-time position with a schedule from 3:00 PM to 11:30 PM, working five 8-hour shifts per week, including weekends and holidays.

Job Description

The Licensed Practical Nurse (LPN) will demonstrate the knowledge and skills necessary to provide care appropriate to the physical and psychosocial needs of the patients in the assigned unit. The LPN will collaborate with other nursing staff and interdisciplinary team members to ensure optimal patient care and satisfaction.

Responsibilities

• Collect data relevant to the resident's physical and psychosocial condition and communicate findings to the RN and interdisciplinary team.
• Contribute to the development of individualized care plans according to nursing diagnoses and interventions.
• Recommend revisions to care plans based on evaluations of resident outcomes.
• Participate in the mentorship and orientation of new staff members.
• Maintain current knowledge and competency in nursing practices through continuing education.

Required Experience / Certifications / Licensure

• Active New York LPN Licensure
• Basic Life Support (BLS), AHA or RQI only
• Minimum of 1 year experience as an LPN, preferably in a Long-Term Care facility
