Physical Therapist - Home Health

This is a 13-week contract position.

Schedule

Shifts are scheduled from Monday to Friday, 8 AM to 5 PM. A flexible schedule may be accommodated if needed.

Job Description

The Physical Therapist will work in a home health setting, providing evaluations and implementing treatment plans for patients. The role requires collaboration with a small team and the ability to manage a varied caseload effectively.

Responsibilities

• Evaluate patients and develop personalized treatment plans.
• Implement therapy interventions to enhance patient mobility and functionality.
• Maintain accurate documentation using Meditech EMR system.
• Manage a caseload of 8 to 11 patients per day, considering travel time.
• Adhere to productivity expectations of 4 to 5 visits per day.

Required Experience / Certifications / Licensure

• Valid Missouri Physical Therapist state license (PT compact privileges possible).
• BLS certification required.
• At least 1 year of experience in physical therapy is required.
