Home Health Registered Nurse (RN)

This position follows a full-time schedule from Tuesday to Saturday.

Job Description

The Home Health RN is responsible for delivering individualized, goal-oriented nursing care in accordance with physician orders. This role also acts as Case Manager when leading cases, ensuring the provision of high-quality home health and hospice care to individuals from diverse backgrounds, regardless of their financial circumstances.

Responsibilities

• Provide comprehensive nursing care to patients in their homes.
• Develop and implement individualized care plans based on patient needs.
• Serve as a Case Manager for patients as the lead discipline.
• Monitor patient progress and adjust care plans as necessary.
• Collaborate with other healthcare professionals to coordinate care.

Required Experience / Certifications / Licensure

• Current RN license in the state of practice.
• Experience in home health or hospice care preferred.
• Strong communication and interpersonal skills.
