Tuesday - Saturday from 8 AM to 4 PM with on-call rotation required.

Job Description

We are seeking a compassionate and skilled Registered Nurse to provide hospice care for our patients. The ideal candidate will be responsible for delivering high-quality nursing care to patients in their homes while ensuring compliance with agency policies and federal regulations.

Responsibilities

• Complete OASIS documentation accurately and in a timely manner.
• Assess patient and caregiver needs using the nursing process.
• Develop and communicate individualized care plans, involving the patient and caregivers.
• Manage caseloads effectively, ensuring agency productivity standards are met.
• Implement medical plans of care and nursing interventions to ensure patient safety and quality of care.
• Maintain up-to-date skills in high-tech care procedures, including IV therapy and wound care.
• Collaborate with LPNs, delegating tasks as appropriate and reviewing documentation.
• Evaluate the effectiveness of care plans and modify as necessary.
• Educate patients and caregivers on medication, treatment options, and discharge instructions.
• Coordinate interdisciplinary needs and communicate with team members effectively.
• Complete necessary agency, state, and federal forms in a timely manner.
• Supervise and assess needs for home caregiver assistance.
• Manage patient discharges or transfers and ensure proper documentation.
• Participate in monthly peer review audits and constructive feedback sessions.
• Facilitate case conferences and ensure effective communication within the team.

Required Experience / Certifications / Licensure

• Current RN license in the state of Vermont.
• Experience in hospice or home health care preferred.
• Knowledge of state and federal nursing regulations.
