Schedule

5x8 Day Shift: 07:30 AM - 04:00 PM. Flexibility in shift times is required, which may include variations from 07:00 AM to 05:00 PM.

Job Description

We are seeking an experienced Pediatric Case Manager RN to join our dynamic healthcare team. This role requires strong case management skills and experience working with pediatric populations. The nurse will facilitate coordinated care for patients from admission through discharge, ensuring all aspects of patient care align with clinical best practices and regulatory standards.

Responsibilities

• Conduct assessments and develop individualized care plans for pediatric patients.
• Collaborate with multidisciplinary teams to coordinate comprehensive care.
• Utilize Epic for charting and documentation of patient care activities and progress.
• Monitor patient outcomes and adjust care plans as needed based on evidence-based guidelines.
• Engage with families to provide education and support regarding care processes and procedures.

Required Experience / Certifications / Licensure

• Active AK RN License required; license must be in hand for auto-offer.
• Minimum of 2 years of Pediatric nursing experience required.
• ACM certification is preferred.
• Experience with Epic electronic medical records is mandatory.
