Licensed Practical Nurse (LPN) - Full Time

Schedule

3:00 PM - 11:30 PM, 5 days a week with every other weekend and holiday rotation.

Job Description

The Licensed Practical Nurse (LPN) will provide comprehensive nursing care tailored to the needs of adult and geriatric patients. This role requires adherence to established protocols and collaboration with the nursing team to ensure effective and safe patient care.

Responsibilities

• Collect and assess data relevant to residents' physical and psychosocial conditions and communicate findings to the Registered Nurse and interdisciplinary team.
• Contribute to the development of culturally relevant care plans that involve the patient and family.
• Recommend revisions to care plans based on resident outcomes and ensure all aspects of care are addressed.
• Maintain current knowledge and competency in nursing practices through ongoing education and in-service programs.
• Participate in mentoring and orientation of new nursing staff, under the guidance of the Registered Nurse.

Required Experience / Certifications / Licensure

• New York LPN licensure.
• Current BLS certification (AHA or RQI).
• Minimum of 1 year of experience as an LPN, preferably in a long-term care facility.
