Licensed Practical Nurse (LPN) - Evening Shift

This is a 40-hour per week position working from 3:00 PM to 11:30 PM, including every other weekend and holiday.

Job Description

The primary functions of a Licensed Practical Nurse (LPN) include assisting with assessment, planning, implementing, and evaluating the care of all patients, regardless of age, during assigned shifts. The LPN is responsible for meeting established standards of skilled nursing practice and collaborates with the care team, participates in interdisciplinary care, and advocates for patients. This role effectively provides Person-Centered care and services, administers medications and treatments under the supervision of a Registered Professional Nurse, and maintains clinical competence while ensuring ethical and professional practice.

Responsibilities

• Monitor and report changes in patients' vital signs including respiration, temperature, blood pressure, and heart rate.
• Document care and observations thoroughly in the EMR/Point Click Care system.
• Coordinate care with the interdisciplinary team to ensure comprehensive patient management.
• Advocate for patients by addressing their needs and concerns in a professional manner.
• Assist in maintaining a safe environment for residents.
• May be required to float to other units and possibly perform duties as a Certified Nursing Assistant (CNA) as staffing allows.

Required Experience / Certifications / Licensure

• Valid LPN license as required by state regulations.
• Experience with Point Click Care is preferred.
• Completion of required health screenings and immunizations.
