Licensed Practical Nurse (LPN) - Evening Shift

Job Description

The primary functions of an LPN include assisting with the assessment, planning, implementation, and evaluation of patient care across all ages. The role is responsible for ensuring that established standards of skilled nursing practice are met. The Licensed Practical Nurse collaborates with the care team, engages in interdisciplinary care, and advocates for patients while effectively providing Person Centered care and services.

Responsibilities

• Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy.
• Takes and records vital signs including respiration, temperature, blood pressure, and heart rate; reports changes in patients' conditions.
• Practices ethically and professionally within the scope of practice; advocates for patients.
• Maintains clinical competence specific to the care area and participates in required education within the stated timeframe.
• Coordinates care with the interdisciplinary team and documents appropriately and thoroughly in EMR/Point Click Care.
• Informs other nursing personnel and the responsible RN regarding any changes in the condition of patients/residents and other pertinent observations.

Schedule

This position requires work during the evening shift from 3:00 PM to 11:30 PM, with every other weekend and holiday shifts mandatory.

Required Experience / Certifications / Licensure

• Current and valid LPN license.
• Experience with Point Click Care preferred.
• Must be willing to float to other units as staffing permits.
• May be required to work in a CNA position if necessary.
