Care Coordinator - 8-Hour Shifts

Job Description

The Care Coordinator is responsible for coordinating the care and services for assigned patients during their episode of illness or treatment. This role involves collaboration with the patient, family, physician, patient care team, and payors. Utilizing advanced clinical skills, the Care Coordinator facilitates the provision of care through assessment, planning, intervention, and evaluation principles of the nursing process. The Care Coordinator also promotes efficient resource utilization while maintaining optimal clinical outcomes.

Responsibilities

• Coordinate care and services for assigned patients in collaboration with multidisciplinary teams.
• Utilize the nursing process for assessment, planning, intervention, and evaluation of patient care.
• Advocate for patients and families throughout the entire episode of care.
• Monitor and evaluate quality/utilization cases, ensuring compliance with admission/discharge criteria.
• Participate in outlier review on an ongoing basis.
• Assist in the Emergency Department as needed, ensuring efficient and cost-effective care.
• Initiate appropriate clinical pathways based on admission diagnosis for utilization management purposes.
• Maintain communication with patients and families as a resource for care coordination.

Required Experience / Certifications / Licensure

• At least one year of recent Case Management experience in an acute care setting.
• Current CA Licensure.
• Current CPR recognition.

Schedule

8-hour shifts from 8am to 4:30pm, including weekends and weekdays.

Assignment Duration

This is a 13-week contract position.
