5x8.0 schedule with 1-2 evenings until 7 PM.

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on patients' needs, goals, and physician direction. The therapist is responsible for delegating and directing all discipline-specific therapy services for assigned patients. This position ensures the design of functional treatment programs facilitating effective return to community activities, while serving as an educator for patients, families, students, coworkers, and self. The therapist upholds the organization's Mission Statement and Core Values, maintaining compliance with the organizational Code of Conduct, including confidentiality regarding patient and company information.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Document medical records accurately and within agreed timelines, adhering to state and federal guidelines.
• Participate in the student program, providing supervision and evaluation.
• Create individualized professional development plans and pursue continuing education.
• Adhere to discipline-specific regulations, maintaining necessary licensure and ethical standards.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR Certification.
• Active and applicable state license.
