LPN - Full Time Day/Night Rotations

Full-time Day/Night rotations (5x8s). Must be able to start on a Tuesday or Thursday for full-shift orientation.

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, under the supervision of a Registered Nurse. The LPN will demonstrate knowledge of principles related to patient care across the life span and will engage in maintaining collaborative relationships with other healthcare staff.

Responsibilities

• Collect and communicate data relevant to resident's physical and psychosocial condition
• Contribute to the development of age-appropriate and culturally relevant care plans
• Recommend revisions to care plans based on evaluations of resident outcomes
• Engage with residents/families and interdisciplinary team members to achieve desired outcomes
• Maintain current knowledge and competency in nursing practice
• Participate in the mentoring and orientation of new staff

Required Experience / Certifications / Licensure

• New York LPN Licensure
• BLS certification through AHA or RQI
• At least 1 year of experience as an LPN, preferably in a long-term care facility
• Ability to provide care to geriatric residents
