Occupational Therapist for Home Health
Location
Missoula, MT
Assignment Duration
This is a 13-week contract position.
Schedule
Days M-F, typically from 8 AM to 5 PM. Occasionally evenings or weekends may be required.
Job Description
The Occupational Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including the management and supervision of patient care as well as patient and family education.
Responsibilities
- Evaluate and establish a plan of care to meet patients' needs in the home environment.
- Coordinate care with other disciplines and seek orders for appropriate additional services.
- Provide direct patient care based on the established care plan and physician orders.
- Apply knowledge of occupational therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation.
- Modify treatment as appropriate, including frequency and duration of care.
- Master scoring of the CMS required OASIS and collaborate with the Quality team for needed adjustments.
- Acquire and maintain a basic understanding of insurance requirements in Home Health.
- Participate in multidisciplinary conferences to establish appropriate patient-relevant care plans.
- Communicate appropriately with physicians and multidisciplinary members.
- Document all skilled services provided to patients, adverse events, and relevant communications in the medical record.
- Participate in Quality Assurance Performance Improvement initiatives.
- Manage and meet productivity standards in line with the Agency's standards.
- Oversee services provided by the Occupational Therapy Assistant and perform supervisory visits within the required time frame.
- Participate in relevant care conferences and plan admissions, OASIS time points, and discharges throughout the patient's episode of care.
- Maintain licensure requirements, including continuing education standards.
Required Experience / Certifications / Licensure
- BS degree or higher from an accredited Occupational Therapy program.
- Current license to practice in the State of Montana.
- Valid driver’s license and appropriate auto insurance.
- Two years of previous OT experience preferred.
