Day shift with rotating weekends.

Job Description

The Hospice teams support patients and family members during the final stages of their illness. The facility provides symptom management, respite care, and room and board, along with skilled nursing care, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, bereavement support, and appropriate medication and equipment.

Responsibilities

• Provide skilled nursing care and symptom management to hospice patients.
• Collaborate with an interdisciplinary team to develop and implement care plans.
• Support and educate patients and families regarding end-of-life issues.
• Offer bereavement support and assistance to families following the patient's passing.
• Maintain accurate and timely documentation in accordance with regulations.

Required Experience / Certifications / Licensure

• Current unencumbered Oregon RN Licensure required upon submission.
• Healthcare Provider Level BLS certification required.
• BSN and/or previous hospice or case management experience preferred.
• Broad-based nursing experience preferred to ensure specialized skills in hospice care.
