Licensed Practical Nurse (LPN) - Full Time Evenings

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents while demonstrating knowledge of age-appropriate care and collaborating effectively with the interdisciplinary team. This position requires a commitment to maintaining high standards of nursing practice and patient care.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial condition and communicate findings to the RN and healthcare team.
• Contribute to the creation of culturally relevant resident care plans reflecting nursing diagnoses and interventions.
• Evaluate resident outcomes and recommend revisions to the care plan, ensuring all aspects of care are addressed.
• Participate in the mentoring and orientation of staff, maintaining current knowledge in nursing practice.
• Demonstrate leadership under the direction of the RN and ensure customer satisfaction within the scope of nursing practice.
• Provide care to geriatric residents while considering safety, cost, and impact in care delivery.
• Maintain compliance with defined policies and procedures, ensuring occupational safety and exposure guidelines are adhered to.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (AHA or RQI only)
• Minimum of 1 year experience as an LPN in a Long-Term Care (LTC) facility

Schedule

Full-time evenings, 5x8 shifts; must be available for full-shift orientation on a Tuesday or Thursday. Rotations may include days, evenings, nights, and holidays as required.
