Job Description

The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation. Responsibilities

• Identify patient problems affecting their ability to adapt to treatment plans.
• Assist patients and families in utilizing hospital resources.
• Collaborate with the interdisciplinary team to manage clinical care plans and ensure safe discharges.
• Document patient care and discharge planning evaluations accurately and timely.
• Maintain an up-to-date community resource system for patient referrals.
• Ensure compliance with payor requirements for timely certifications and reimbursements.
• Participate in team meetings and enhance operational efficiency.

Required Experience / Certifications / Licensure

• Current licensure in a clinical discipline per state guidelines (RN, RT, SW preferred).
• Associates Degree in a healthcare field (Bachelor’s degree preferred).
• Minimum of 2 years of experience in Case Management and Discharge Planning in a Rehabilitation Care setting.
• Working knowledge of the insurance industry and government reimbursement processes.
