8:30 AM - 5:00 PM, with expectation to work every other weekend and holidays in rotation with hospital employees.

Job Description

Join a dynamic team as a Registered Nurse specializing in Case Management. You will be responsible for overseeing patient care, performing complex discharge planning, and ensuring effective case management practices. This position requires expertise in utilizing EMR systems, specifically Meditech and Midas.

Responsibilities

• Manage patient case loads effectively.
• Develop and implement discharge plans.
• Coordinate care with multidisciplinary teams.
• Ensure compliance with hospital policies and procedures.
• Utilize EMR systems for documentation and reporting.

Required Experience / Certifications / Licensure

• Active RN license.
• Experience with Meditech and Midas systems.
• Strong background in case management and discharge planning is preferred.
