Shift: 9:00 AM - 5:30 PM with weekend rotation.

Job Description

We are seeking a dedicated Travel RN Case Manager to join our team. The ideal candidate will have experience with Meditech and MIDAS, and will be responsible for Acute Care Case Management involving Utilization Review, Discharge Planning, and Transitions of Care, including Home Health and Skilled Nursing Facility setups.

Responsibilities

• Conduct assessments and provide recommendations for discharge planning.
• Collaborate with multidisciplinary teams, including rounding with medical teams.
• Ensure compliance with InterQual criteria.
• Manage transitions of care effectively to optimize patient outcomes.

Required Experience / Certifications / Licensure

• Must possess a valid RN license.
• Experience with DCP Assessment Skills and case management.
• Familiarity with Meditech and MIDAS documentation systems.
