Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on the patient’s needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of effective treatment programs for a successful return to community activities.

Responsibilities

• Completes comprehensive evaluations and develops appropriate treatment plans.
• Documents all medical records in a clear, concise, and timely manner.
• Participates in student programs, providing mentorship and evaluations.
• Adheres to discipline-specific State Practice Act and maintains professional licensure.
• Implements and participates in performance improvement initiatives.
• Promotes patient and employee safety in all service areas.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 6 months to 1 year of outpatient clinical experience is required.
• Current CPR certification is required.
• Active and applicable state licensure for physical therapy is required.
