Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient’s needs, goals, and physician direction. This role involves the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that allow for an effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and self, while also performing other professional duties associated with patient care and program delivery. Consistent adherence to the organization's Mission Statement and Core Values is expected.

Responsibilities

  • Completes a comprehensive evaluation of assigned patients.
  • Utilizes clinical reasoning to develop appropriate treatment plans and goals.
  • Documents medical records accurately and timely, in compliance with regulations.
  • Provides care under valid physician orders and maintains compliance with insurance approvals.
  • Participates in the student program, providing effective supervision and evaluation.
  • Develops a professional growth plan and pursues continuing education.
  • Maintains professional licensure and adheres to ethical standards.
  • Demonstrates awareness of and participates in performance improvement initiatives.

Required Experience / Certifications / Licensure

  • Degree in Physical Therapy from an accredited school.
  • 6 months to 1+ year of outpatient clinical experience is required.
  • Current CPR certification.
  • Active/state licensure for physical therapy.

Schedule

This position offers a 5x8.0 schedule with 40 hours per week, starting August 10th. The model includes a 60-minute evaluation and follow-ups every 30 minutes.
