Full Time Nights, 5x8 per week. Weekends and Holidays required.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care for assigned residents. The LPN works under the supervision of a Registered Nurse and maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff. The role involves adhering to defined policies and procedures while demonstrating the knowledge and skills necessary to provide age-appropriate care for adult and geriatric patients.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans reflecting nursing diagnoses and interventions aimed at achieving desired resident outcomes.
• Evaluate resident outcomes and recommend revisions to the care plan, indicating the need for referrals as necessary.
• Participate in mentoring and orientation of staff while maintaining current knowledge and competency in nursing practice.
• Provide leadership under the direction of the RN while considering factors related to safety and cost in the delivery of care.
• Demonstrate awareness of occupational exposure to blood-borne pathogens and undertaking appropriate precautions.

Required Experience / Certifications / Licensure

• Valid New York LPN Licensure.
• Basic Life Support (BLS) certification through AHA or RQI.
• At least 1 year of experience as an LPN, preferably in a Long-Term Care (LTC) facility.
