LPN Full Time Day/Night Rotation (5x8)

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents in accordance with defined policies and procedures under the supervision of a Registered Nurse. This position requires a focus on both the physical and psychosocial aspects of resident care, particularly for adult and geriatric patient groups.

Responsibilities

• Collect ongoing data relevant to the resident's physical and psychosocial condition and communicate findings effectively to the Registered Nurse and other interdisciplinary team members.
• Contribute to the development of a culturally relevant resident care plan that reflects nursing diagnoses and interventions aimed at achieving desired outcomes.
• Assess and revise care plans as necessary, ensuring comprehensive care for all aspects of the resident's health needs and promoting a safe and healthy environment.
• Maintain knowledge and competency in nursing practice, participating in mentorship and orientation of new staff.
• Provide leadership within the nursing practice setting under the direction of a Registered Nurse.
• Manage occupational exposure to blood borne pathogens while fulfilling assigned tasks.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure
• Basic Life Support (BLS) certification through AHA or RQI
• Minimum of 1-year experience as an LPN, specifically in a long-term care facility

Schedule

This position requires day and night rotations, working 5 days a week, each shift being 8 hours.
