Licensed Practical Nurse (LPN) - Full Time

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care within a supportive environment. Under the direction of licensed nursing personnel, the LPN will perform both direct and indirect patient care, ensuring that all residents receive the care and support they need in accordance with their individualized care plans.

Responsibilities

• Assist with residents' Activities of Daily Living (ADLs) such as hygiene, grooming, and dressing.
• Maintain a safe and clean environment, responding promptly to resident calls and alarms.
• Ensure residents' nutritional needs are met in line with their care plans, including meal delivery and feeding assistance.
• Facilitate rehabilitative needs as outlined in care plans, including range of motion and ambulation support.
• Report changes in residents' physical and emotional status to the Charge Nurse promptly.

Required Experience / Certifications / Licensure

• Current New York LPN License.
• BLS certification (AHA or RQI only).
• Minimum of one year of experience as an LPN, preferably in long-term care or as a certified nursing assistant.
• High School Diploma or GED.

Schedule

Full-time position with required availability for holidays. Must be able to start on a Tuesday or Thursday for full shift orientation.
