Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care for a designated group of patients. The LPN must demonstrate the knowledge and skills necessary to care for the adult and geriatric patient populations. The position requires collaboration with other nursing staff, physicians, and ancillary staff while following defined policies and procedures under the supervision of a Registered Nurse.

Responsibilities

• Collect and assess data relevant to the patient's physical and psychosocial condition on an ongoing basis.
• Contribute to the development of individualized care plans reflecting nursing diagnoses and interventions.
• Recommend necessary revisions to the care plan based on patient outcomes.
• Collaborate with the healthcare team to ensure comprehensive patient care.
• Maintain current knowledge and competency in nursing practice, including mentoring staff.
• Demonstrate leadership within the nursing practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• Basic Life Support (BLS) Certification
• Minimum of 1 year of experience as an LPN, preferably in a long-term care facility
• Ability to provide care to geriatric residents

Schedule

Candidate must be available to start on a Tuesday or Thursday, working a full shift. A minimum of 24 hours over 4 weeks is required, including 2 weekend shifts.
