Licensed Practical Nurse (LPN)

Schedule

Candidate must be available to start on a Tuesday or Thursday, full shift, with a minimum requirement of 24 hours per 4 weeks, including 2 weekend shifts.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care to assigned residents under the supervision of a Registered Nurse. The LPN must possess knowledge of growth and development appropriate to the age of patients, particularly within the adult and geriatric populations. This role requires the ability to assess patient data and provide care tailored to each patient’s specific needs.

Responsibilities

• Collect ongoing data related to residents' physical and psychosocial conditions and communicate findings to the RN and the interdisciplinary team.
• Contribute to the development of culturally relevant care plans reflecting nursing diagnoses and interventions with desired outcomes.
• Recommend revisions to care plans based on evaluations of resident outcomes, ensuring comprehensive care through collaboration with residents, families, and staff.
• Maintain current knowledge and competency in nursing practice and participate in mentoring and orientation of staff.
• Provide leadership in the professional practice setting under the direction of an RN, ensuring safe and cost-effective care delivery.

Required Experience / Certifications / Licensure

• Active New York LPN Licensure
• Basic Life Support (BLS) Certification
• Minimum of 1 year of experience as an LPN in a long-term care facility
