RN Case Manager

Job Description

We are seeking a skilled Registered Nurse Case Manager to coordinate management of care, ensuring optimum utilization of resources and compliance with external review agencies. The ideal candidate will provide ongoing support and expertise through comprehensive assessment, care planning, implementation, and overall evaluation of individual patient needs. This role is essential to enhance the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness.

Responsibilities

• Coordinate management of patient care and optimize resource utilization.
• Conduct comprehensive assessments and develop detailed care plans.
• Implement care plans and continuously evaluate the needs of individual patients.
• Advocate for patients, addressing their holistic needs through resource utilization and discharge planning.
• Collaborate with interdisciplinary teams to enhance service delivery and patient outcomes.

Required Experience / Certifications / Licensure

• Minimum of 5 years recent RN Case Management experience, preferably in LTAC or STACH settings.
• Experience with MCG and discharge planning across acute care settings.
• Strong collaboration skills with interdisciplinary teams.
• Current California RN license.
• BLS certification through the American Heart Association.

Schedule

This position operates Monday to Friday from 8:00 AM to 5:00 PM. This is a 13-week contract position.
