Job Description

The Acute Care Case Manager is responsible for case management and discharge planning for hospitalized patients. The role includes assessing patients, coordinating care, and ensuring smooth transitions to post-acute settings. The professional will engage in family meetings and contribute to the development of treatment plans while adhering to regulatory guidelines.

Responsibilities

• Assess patients for care coordination and discharge planning.
• Organize and facilitate family meetings concerning patient care.
• Develop and implement treatment plans in collaboration with interdisciplinary teams.
• Ensure compliance with CMS regulations and The Joint Commission standards.
• Coordinate post-discharge services, including DME and SNF placement.
• Participate in utilizations reviews and needs assessments.

Required Experience / Certifications / Licensure

• Must have 2 years of experience in Acute Care Case Management and Discharge Planning.
• Hospital experience is required; behavioral health experience does not apply.
• California State RN License required; pending licenses accepted but must be active upon start.
• Experience with Epic charting system is required.
• Must commit to working every other weekend and 3 of the 5 major holidays.
