Job Description

As a Registered Nurse providing care through home health services, you will deliver a full range of professional nursing services to clients of all ages. Your role will encompass admission processes, evaluations, and the development of tailored care plans. Working collaboratively with a dedicated team of homecare professionals, you will ensure the highest level of coordinated care for clients throughout Caledonia and southern Essex counties.

Responsibilities

• Conduct patient assessments and admissions.
• Develop and implement individualized care plans.
• Administer medications and treatments as prescribed.
• Collaborate with paraprofessionals and other healthcare team members.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on health conditions and care plans.

Required Experience / Certifications / Licensure

• Current Vermont Registered Nurse (RN) license.
• Minimum of one year of prior clinical experience required.
